Health Services

Eastbrook Community Schools provides students with a school nurse to administer medications, care for acute illness or injury during school hours, and accommodate students with medical conditions as needed. If you have specific requests on concerns regarding your student, please reach out to the nurse at your student’s school. These will need to be updated and signed for EACH school year.

Care plans are required for students with certain health conditions. These are to be updated each school year prior to the start of school. Some care plans will need to be signed by your child’s doctor. These documents will be available at school clinics and online.

If your child has MEDICATIONS TO BE GIVEN AT SCHOOL, there is a medication admin form that needs to be filled out for each medication with specific instructions. Medications must be provided in original prescription bottles with your child’s name. If there is an OTC medication to be given, we must have a doctor’s order to administer.

If your child has SEIZURES/EPILEPSY, they will need a seizure action plan. This will be filled out by a physician. If there are any changes in activity, please keep the nurse updated.

If your child has ASTHMA, they will need an asthma action plan. This document will be filled out by your child’s doctor so we know exact treatments to follow. Inhalers must be available at school for children with asthma. **If your child is old enough and able to carry and self administer, we must have a doctor’s note stating the student may carry and treat themselves as needed.

If your child has ANAPHYLAXIS that requires the use of an EPIPEN, they will need an allergy action plan. This medication must be available at the school at all times. This form will tell us an exact treatment plan to follow. **If your child is old enough and able to carry and self administer, we must have a doctor’s note stating the student may carry and treat themselves as needed.

If your child is DIABETIC, they will need a current diabetic care plan from their doctor. If any changes occur, please contact the nurse immediately and provide new documentation. The district nurse will set up a meeting regarding diabetic care yearly. **All extra supplies need to be available at the school in case of emergency. Students will need ketone sticks, glucagon/fast acting carbs, snacks, juices, etc provided by the parents and kept in a student specific bin in the nurse’s office. These will be tailored to specific student needs.

If your child has FOOD ALLERGIES, such as NUTS, EGGS, SOY, SHELLFISH, ETC, please make the school nurse aware of the severity and treatment.

If your child has any other health conditions not listed above, please reach out to the school nurse to discuss this and any accommodations needed for your student. Please be prepared to provide documentation from your child’s doctor regarding this and any steps needed to manage the condition at school.
